مرض الكلى السكري (DN) هو مضاعفة تتعلق بالكلى تصيب حوالي 40% من مرضى السكري من النوع 1 والنوع 2، ويُعتبر أحد أكثر الأسباب شيوعًا للفشل الكلوي المزمن (ESRD) على مستوى العالم. على الرغم من استخدامه حاليًا كجزء من تشخيص مرض الكلى السكري، إلا أن هناك جدلًا كبيرًا بشأن دور الألبومينوريا كنقطة نهاية بديلة للمرض الكلوي المزمن. بالإضافة إلى ذلك، أشارت دراسات أخرى إلى أن الميكروالبومينوريا ليست متنبئة دقيقة بخطر مرض الكلى السكري نظرًا لأنها تظهر تقلبات يومية كبيرة يمكن أن تتأثر بظروف أخرى مثل ممارسة التمارين الرياضية وأنواع معينة من الحميات الغذائية والعدوى وارتفاع ضغط الدم. تم اقتراح إكزوزومات البول كمؤشر تشخيصي محتمل لأمراض الكلى المزمنة. في دراستنا، نهدف إلى تحليل محتوى الـ miRNA والبروتين في إكزوزومات البول لدى مرضى مرض الكلى السكري في مراحل المرض المختلفة وتقييم استخدامه كمؤشر تشخيصي وتوقعي لمرض الكلى السكري. توفر وجود مؤشر حساس ومعلوماتي لمرض الكلى السكري الذي يمكنه الكشف المبكر عن المرضى الذين سيتقدمون إلى ESRD سيسمح بالتدخل المبكر. بالإضافة إلى ذلك، مع تطوير علاجات جديدة لمرض الكلى السكري، هناك حاجة كبيرة لمؤشرات تقدم المرض يمكن استخدامها لتقييم استجابة العلاج.
Abstract English
Diabetic nephropathy (DN) is a kidney-related complication that affects around 40% of type 1 and type 2 diabetic patients and is regarded as one of the most common causes of end-stage renal disease (ESRD) worldwide. Despite currently being used as part of DN diagnosis, there is a great controversy over albuminuria’s role as a surrogate end-point for the chronic kidney disease. In addition, other studies pointed that micro-albuminuria is not a precise predictor of DN risk as it showed considerable daily variation as its levels can be influenced by other conditions including exercise, certain types of diets, infections and high blood pressure. Urinary Exosomes have been proposed as a potential diagnostic biomarker for chronic kidney diseases. In our study, We aim to analyze the miRNA and protein content of urinary exosomes in DN patients at different disease stages and evaluate its use as a potential biomarker for DN diagnosis and prognosis. The availability of a sensitive and informative biomarker for DN that can early detect patients who will progress into ESRD would allow early intervention. In addition, as novel therapies for DN are being developed, there is a huge need for markers of disease progression that can be used to evaluate therapy response.
