سرطان الثدي الثلاثي السلبي (TNBC) هو نوع فرعي هائل من سرطان الثدي يتميز بغياب مستقبلات هرمون الاستروجين (ER)، ومستقبلات البروجسترون (PR)، ومستقبل عامل نمو البشرة البشري 2. (HER2) هذا النقص المتأصل في الأهداف العلاجية جعل TNBC مقاومًا للعلاجات التقليدية. إن الطبيعة العدوانية لـ TNBC، إلى جانب الخيارات العلاجية المحدودة، استلزمت استكشاف أساليب مبتكرة، وأبرزها العلاج المناعي.

Abstract English
Triple-negative breast cancer (TNBC) is a formidable breast cancer subtype characterized by the absence of estrogen receptor (ER), progesterone receptor (PR), and human epidermal growth factor receptor 2 (HER2) expression. This inherent lack of therapeutic targets has rendered TNBC resistant to conventional treatments for other breast cancer subtypes. The aggressive nature of TNBC, coupled with limited therapeutic options, has necessitated the exploration of innovative approaches, most notably immunotherapy. Pembrolizumab, a monoclonal antibody targeting the programmed cell death protein 1 (PD-1), has demonstrated remarkable efficacy across various malignancies by reinvigorating exhausted T cells and reinstating antitumor immune responses. Key clinical trials, exemplified by the KEYNOTE-086 and KEYNOTE-119 studies, have furnished critical evidence regarding pembrolizumab's effectiveness in advanced TNBC.
Nonetheless, challenges persist as not all TNBC patients respond favorably to pembrolizumab, underscoring the need for predictive biomarkers to identify suitable candidates for this therapy. In response to this imperative, our project scrutinizes the TNBC immune microenvironment. Our analysis will encompass specific immune cell markers, including CD45 (a pan-leukocyte marker), CD8 (a cytotoxic T-cell marker), CD56 (an NK-cell marker), CD68 (a macrophage marker), CXCL-9 (a T-cell chemokine), and microRNAs like miR-21, known to promote TNBC proliferation and invasion. We hypothesize that a composite evaluation of these immune microenvironment components will yield valuable correlations with the pembrolizumab response.
