المرحلة 1
الهدف: دراسة تأثير إضافة العلاج المركب من داباجليفلوزين مع بياوجليتازون إلى الأنسولين على التحكم في مستوى السكر في الدم وتركيز الكيتونات في البلازما لدى المرضى المصابين بداء السكري من النوع 1 (T1DM).
تصميم البحث: يتكون عدد 120 مريضاً مصاباً بداء السكري من النوع 1 والذين هم بصحة جيدة من مجموعة الدراسة. بعد الفحص، سيبدأ الأفراد المؤهلون فترة اختبار مدتها 4 أسابيع. في الأسبوع الرابع، سيبدأ الأفراد في تلقي داباجليفلوزين لمدة 12 أسبوعًا. في الأسبوع 16، سيتم توزيع الأفراد عشوائيًا لتلقي بياوجليتازون أو دواء وهمي بطريقة مزدوجة التعمية لمدة 16 أسبوعًا.

Abstract English

Phase 1
Purpose: To examine the effect of addition of combination therapy with dapagliflozin plus pioglitazone to insulin on glucose control and plasma ketone concentration in patients with type 1 diabetes (T1DM)
Research Design: 120 patients with type 1 diabetes who otherwise are healthy constitute the study population. After screening, eligible subjects will start 4 week run in. At week 4, subjects will receive dapagliflozin for 12 weeks. At week 16, subjects will be randomized to receive in a double blind fashion pioglitazone or placebo for 16 weeks.
Methods: the following techniques will be employed in the present study: (1) mixed meal tolerance test; (2) indirect calorimetry; (3) continuous glucose monitoring.
Clinical Relevance: the results of the present study will demonstrate that the addition of pioglitazone to SGLT2 inhibitor in T1DM patients produces greater reduction in the HbA1c without increasing risk of ketoacidosis and hypoglycemia.
Phase 2: Extension
Our preliminary results confirm our working hypothesis and demonstrate that in T1DM patients receiving therapy with dapagliflozin plus insulin in a dose >0.5 units/kg body weight, the addition of pioglitazone causes a further significant reduction in the HbA1c and reverses the increase in plasma ketone concentration caused by dapagliflozin. Further, they also demonstrate that the combination of pioglitazone plus SGLT2 inhibitor can safely be used in T1DM, not only for metabolic control, but also for a cardio-renal outcome study in T1DM patients who have been excluded from cardiorenal protection studies with SGLT2 inhibitors.
As stated above, the decrease in HbA1c in subjects in Group 1 did not reach a steady state at the End of Study (EOS) (week 32). Thus, the present interim analysis may not demonstrate the full metabolic benefit of pioglitazone in T1DM patients receiving SGLT2 inhibitor, and it will be of great interest to extend therapy with pioglitazone plus dapagliflozin beyond week 32. Because the mechanism of pioglitazone’s action to improve glucose control is mediated via changes in gene expression, it takes a relatively long time (>16 weeks) for the effect of pioglitazone on HbA1c to become fully evident (1). In long term studies in patients with T2DM (2-4), it took 24-36 weeks for the decrease in HbA1c caused by pioglitazone to reach a steady state (Figure 5). Therefore, we suggest extending the treatment with pioglitazone plus dapagliflozin in the present study for an additional 24 weeks. This time period will allow the decrease in HbA1c in patients in Group 1 to reach steady state. Further, it also allows sufficient time for the decrease in HbA1c in patients in Group 2, who will be switched from dapagliflozin plus placebo to dapagliflozin plus pioglitazone, to reach a steady state.
