نظرًا لأن التفاعلات بين الجينات والبيئة تعتبر من العوامل الرئيسية في مسببات داء السكري (DM)، فإن المفتاح لنجاح جهودنا في تحقيق مهمة معهد الأمراض الوراثية (DDI) هو تحديد العوامل الوراثية المتعلقة بداء السكري والاضطرابات المرتبطة به على مستوى الجينوم. على الرغم من أن العديد من الدراسات العالمية التي تستخدم بيانات الجينوتيب من الأفراد غير المرتبطين قد حددت العديد من المواقع الجينية المتعلقة بداء السكري من النوع الثاني (T2D)، إلا أنها تفسر فقط جزءًا صغيرًا من التوريث الملحوظ (التجمع العائلي) لداء السكري من النوع الثاني.
من بين التفسيرات الممكنة للتوريث المفقود قد يكون دور المتغيرات النادرة والأجزاء الموروثة من التماثل الوراثي. تسمح التصاميم القائمة على العائلات بتجزئة المتغيرات النادرة في سلالة، كما أن النسخ المتعددة من هذه المتغيرات النادرة تسهل اكتشاف تأثيراتها.
تعد الزيجات في المجتمع العربي عادةً زواجًا أقاربيًا، ويؤدي التزاوج القريب عبر الأجيال المتعاقبة إلى زيادة مستويات التزاوج الداخلي، والأليلات المتنحية، ونسبة المناطق الجينية المتماثلة في المجتمع العربي. وغالبًا ما يُلاحظ زيادة في خطر داء السكري من النوع الثاني بين أبناء هذه الزيجات القريبة وتجميع داء السكري في العائلات في المجتمع العربي.
لذلك، يوفر المجتمع العربي إمكانات كبيرة للتصاميم القائمة على العائلات حيث يمكن أن تظهر تجمعات جينية عائلية لداء السكري والخصائص الأيضية. تهدف هذه الدراسة العائلية المعتمدة على شجرة النسب والعائلات الثلاثية إلى تحديد العناصر الجينية النادرة والمؤسسية (مثل مواقع الجينات، SNPs، التغيرات في عدد النسخ [CNVs]، توقيعات التزاوج الداخلي، والتغيرات الجينية الأخرى) التي تكمن وراء (1) تجمع داء السكري من النوع الثاني في العائلات، (2) زيادة خطر داء السكري من النوع الثاني في العائلات القريبة، و (3) الانتشار المرتفع لداء السكري من النوع الثاني في المجتمع العربي ذي التزاوج الداخلي العالي.
من النتائج المهمة لهذه الدراسة هو إنشاء قاعدة بيانات للتغيرات الجينية التي تم رصدها في العائلات الكويتية.
Abstract English
Since gene–environment interactions are implicated as major players in the etiology of DM, the key to the success of our efforts to achieve the DDI’s mission is to identify the genome-wide genetic risk factors for DM and related disorders. Although a large number of global studies using genotype data from unrelated individuals have identified many T2D genetic loci, they explain only a relatively small proportion of observed heritability (familial clustering) of T2D.
Possible explanations for the missing heritability may originate from, among others, the role of rare variants and inherited segments of homozygosity. Family-based designs allow the segregation of rare variants in a pedigree, and multiple copies of such rare variants facilitate the detection of their effects.
Marriages in the Arab population are traditionally often consanguineous, and consanguinity in successive generations cumulatively increases inbreeding levels, recessive alleles, and the proportion of homozygous gene regions in the Arab population. An increased risk of T2D among the offspring of such consanguineous marriages and familial clustering of T2D are often seen in the Arab population.
Thus, the Arab population offers a large potential for family-based designs as it can show familial gene clustering for DM and metabolic traits. This pedigree- and trio-based familial study aims to delineate rare and founder genetic elements (gene loci, SNPs, copy number variations [CNVs], inbreeding signatures, and other genome rearrangements) that underlie (i) the observed familial clustering of T2D, (ii) the observed increased T2D risk in consanguineous families, and (iii) the high prevalence of T2D in the highly consanguineous Arab population.
An important outcome of the study is a database of genetic variations seen in Kuwaiti families.
