لقد وصلت السمنة إلى مستويات وبائية على مستوى العالم، حيث تُشكّل الأمراض المرتبطة بالسمنة العبء الأكبر على الصحة العامة والاقتصاد. وعلى الرغم من زيادة الوعي المجتمعي والاستراتيجيات الوقائية، لا تزال معدلات انتشار السمنة والأمراض المصاحبة لها في ازدياد. ولذلك، تزداد الحاجة الملحة لفهم المسارات التي تؤدي من خلالها السمنة إلى هذه الأمراض، وتطوير استراتيجيات جديدة لمنع تطورها. من المُسلّم به حاليًا أن السمنة تُميَّز بخلل في تنظيم الجهاز المناعي والتهاب مزمن على مستوى الجسم بأكمله (الالتهاب الأيضي). وقد أصبح دور الالتهاب الأيضي في الإصابة بالسكري من النوع الثاني والأمراض المصاحبة له واضحًا، إلا أن المسارات التي تعزز وتحافظ على هذه الحالة الالتهابية لا تزال غير مفهومة بالكامل. لذلك، يجب علينا اكتساب فهم عميق للآليات الجينية التنظيمية التي تدعم النشاط الالتهابي للخلايا المناعية إذا أردنا الحد من انتشار الأمراض الأيضية دون اللجوء إلى كبت مناعي شامل. في سياق التحكم النسخي للجينات، تُعد التنظيمات اللاجينية، مثل أستلة ومثيلة الهيستونات ومثيلة الحمض النووي (DNA)، ضرورية لنسخ الجينات. وقد ثبت أن التوازن الدقيق بين الحالة المُستلَة وغير المُستلَة للكروماتين هو ما يُنظِّم نسخ الجينات. ورغم التقدم في الدراسات المتعلقة بالتعديلات اللاجينية في الخلايا المختلفة المرتبطة بالأمراض الأيضية، إلا أن الآليات التي تحفّز هذه التعديلات في الخلايا المناعية لم تُفهَم بشكل كامل بعد. وبما أن السمنة تترافق بوجود كميات كبيرة من الوسائط الالتهابية (أديبوكينات مثل TNF-α وIL-6) والأحماض الدهنية الحرة في الدورة الدموية، فإننا نفترض أن المشهد اللاجيني غير الطبيعي في الخلايا المناعية، الناتج عن هذه الأحماض والأديبوكينات (بشكل فردي أو تفاعلي)، قد يلعب دورًا رئيسيًا في تعزيز واستدامة الالتهاب في السمنة. سؤال دراستنا هو: هل يؤدي التفاعل بين الأحماض الدهنية الحرة والأديبوكينات إلى إعادة تشكيل المشهد اللاجيني في الخلايا المناعية، بطريقة تروّج للالتهاب المزمن ومقاومة الإنسولين في حالات السمنة؟ سنقوم باختبار فرضيتنا من خلال الأهداف المحددة التالية: 1) تحديد ما إذا كان، وكيف، يؤثر التفاعل بين الأحماض الدهنية الحرة والأديبوكينات على التعديلات اللاجينية. 2) توضيح دور العلامات اللاجينية المُحددة في تسهيل الالتهاب الناتج عن السمنة. 3) تحديد ما إذا كان استهداف التعديلات اللاجينية باستخدام مثبط انتقائي يمكن أن يُحسّن الالتهاب الأيضي والمضاعفات المرتبطة به في الجسم الحي. 4) تحديد ما إذا كانت التغيرات اللاجينية تؤثر على النمط الظاهري للخلايا المناعية البشرية الأولية، وكيف يتم ذلك. المنهجية: سنستخدم منهجين، داخل الجسم الحي وخارجه، تحت تأثير أنظمة غذائية عالية الدهون (HFD) بمصادر دهون مختلفة، مع استخدام الأديبوكينات مثل IL-6 وTNF-α لتحقيق أهدافنا. سيتم عزل الخلايا المناعية (مثل الوحيدات/البلعميات) من الدورة الدموية للفئران ومن الأعضاء الأيضية الرئيسية مثل النسيج الدهني والكبد. كما ستُستخدم عينات من الخلايا وحيدة النواة في الدم المحيطي (PBMCs) وعينات البلازما من أفراد نحيفين، زائدي الوزن، وبدينين من خلال مشروع كاديم (KADEM) متعدد التخصصات لتأكيد نتائجنا. لتحديد العلامات اللاجينية ووظائفها، سنستخدم تقنيات مثل التدفق الخلوي (Flow Cytometry)، المجهر، ChIP-qPCR، وELISA. النتائج المتوقعة والأهمية الصحية: من المتوقع أن توفر هذه الدراسة بيانات ميكانيكية سريرية معمقة تسلّط الضوء على أهمية التنظيم اللاجيني في نشوء الالتهاب الأيضي، وتساعدنا في تطوير أدوية دقيقة لمعالجة الأمراض المرتبطة بالسمنة. كما تهدف دراستنا إلى تحديد مؤشرات حيوية لاجينية للالتهاب الأيضي، تُسهّل عملية التشخيص المبكر والمعالجة الفعالة للأمراض الناتجة عن السمنة.

Abstract English

Obesity has reached epidemic proportions globally, with the biggest health and economic burden being the many obesity-related diseases. Despite increased public awareness and prevention strategies, the prevalence of obesity and related diseases continues. Therefore, there is increased urgency to understand the pathways whereby obesity leads to other diseases and to develop new strategies to prevent their progression. It is now appreciated that obesity is characterized by immune dysregulation and chronic systemic inflammation (metabolic inflammation). The role of metabolic inflammation in type 2 diabetes and its comorbidities is now clear, although the pathways that promote and sustain the inflammatory process remain murky. Therefore, we must obtain deep knowledge of the gene regulatory mechanisms supporting the proinflammatory activity of immune cells if we hope to stem the tide of metabolic disease while also avoiding complete immunosuppression. In the context of transcriptional control of genes, epigenetic regulation, such as histone acetylation and methylation and DNA methylation, is essential for gene transcription. It is well documented that the delicate equilibrium between the acetylated and the deacetylated state of chromatin orchestrates gene transcription. Despite advances in studies concerning epigenetic modifications in various cells in metabolic diseases, the mechanisms that drive these changes in immune cells are not fully understood. Since in the state of obesity, a large number of inflammatory mediators (adipokines; TNF-α, IL-6, etc.) and free fatty acids exist in circulation; we hypothesize that aberrant epigenetic landscapes in immune cells driven by free fatty acids and adipokines (individually or interactively) may play a key role in sustaining and promoting inflammation in obesity. Our study question is: Does crosstalk between FFAs and adipokines remodel epigenetic landscapes in immune cells, which favors chronic inflammation and insulin resistance in obesity? We will investigate our hypothesis through the following specific aims: 1) To determine whether and how the interaction between FFAs and adipokines induces epigenetic modifications. 2) Define the role of identified epigenetic marks in facilitating obesity-induced inflammation. 3) Determine if targeting epigenetic modification by a selective inhibitor can ameliorate metabolic inflammation and associated complications in vivo. 4) To define whether and how epigenetic changes affect primary human immune cells' phenotype. Methodology: We will use both in-vitro and in-vivo approaches under the influence of high-fat diets (HFD) with different sources of fat and adipokines, such as IL-6 and TNF-α, to achieve our objectives. Immune cells (monocytes/macrophages, etc.) will be isolated from the peripheral compartment (circulation) of mice and major metabolic organs, including adipose tissue and the liver. Human PBMCs and plasma samples of lean, overweight, and obese individuals will be used through the multidisciplinary KADEM project to validate our findings. To determine epigenetic markers and their function, we will use flow Cytometry, microscopy, ChIP-qPCR, ELISA, etc. Expected Results and Health Relevance: The proposed studies will generate deep mechanistic and critical clinical data to support the importance of epigenetic regulation in the pathogenesis of metabolic inflammation and help us develop precision drugs to treat obesity-associated diseases. Our studies also aim to identify epigenetic biomarkers of metabolic inflammation that will help in the early diagnosis and treatment of obesity-related diseases.
