Abstract English
Hepatic steatosis is characterized by the increased content of the hepatocellular lipids (HCLs) and is frequently observed in insulin-resistant states as seen in type 2 diabetes mellitus (T2DM). The excess of dietary free fatty acids in the plasma of diabetic individuals, is believed to significantly contribute to HCL accumulation. It is known that the lipotoxic accumulation of HCL in hepatocytes along with the imbalance of pro-inflammatory milieu account for insulin resistance. During the onset of diabetes, metabolic inflammation is associated with diseases including insulin resistance, type 2 diabetes and fatty liver disease. However, the pathways that promote hepatic steatosis and trigger cell death during diabetes remain unclear. Bioactive sphingolipids have emerged as key signaling molecules that regulate a diverse range of cellular processes that are important not only in immunity but also intercellular lipid homeostasis. Ceramide is considered as the central molecule in sphingolipid metabolism which regulates both pro-apoptotic and anti-proliferative processes. Sphingomyelin (SM) hydrolysis is catalyzed by sphingomyelinases (SMases) and it is considered a major pathway for stress-induced ceramide production. The activity of neutral sphingomyelinase-2 (nSMase2) catalyze the conversion of SM to ceramide and phosphocholine at the cytosolic leaflet of plasma membrane (PM). Therefore, nSMase2 is considered to be a strong candidate for producing cytokines through ceramide production. The activation of nSMase-2 was established previously by our lab to be a part of the response of TNF (tumor necrosis factor)-α, a major diabetes related cytokine. The nSMase2 may have an indirect role in lipid accumulation in the liver as the inhibition of ceramide de novo synthesis has been shown to reduce hepatic lipid accumulation in rats with NAFLD. However, little is known about the role of nSMase2 in diabetes related-HCL. Therefore, we hypothesize that this enzyme participates in promoting lipids accumulation, metabolic inflammation, and cell death in hepatic cells. Our goal is to identify the role of SMases in triggering lipotoxicity and cell death under high glucose conditions. Central query: Do SMases (SMPD1, SMPD2 or SMPD3) play a role in regulating lipid accumulation in hepatocytes and cell death; if so, what mechanisms are involved? In reference to SMase transmembrane signaling pathway, ceramide is generated during hydrolysis of sphingomyelin in the cell membranes by nSMase2. In addition to de novo ceramide synthesis, inflammation also upregulates the activity of nSMase2 (3) whereas, nSMase2 deficiency or inhibition by GW4869 alleviates inflammatory responses in vivo (4) and in vitro (5). Ceramides have also been shown to be involved in hepatic steatosis as obese mice with hepatic steatosis were reported to have increased hepatic concentrations of ceramides (6), while, inhibiting de novo ceramide synthesis attenuated the hepatic steatosis, fibrosis, macrovesicular fat accumulation and reduced hepatic triglyceride content (7). Taken together, these evidences establish premise and support our hypothesis that nSMase2 could play a role in regulating lipid accumulation, inflammation, and cell death in the hepatocytes. Preliminary data: Our primary data indicate that nSMase-2 is upregulated in HepG2 cells cultured under hyperglycemic stress conditions. This upregulation was found to be associated with increased cell death by apoptosis. Blocking nSMase-2 enzyme by its inhibitor (GW4869) reduced the cell death while treatment with its agonist (DNR) dramatically increased cell viability. Significance: Our results suggest that nSMase2 may play a major role in regulating the viability of hepatic cells under high glucose conditions. This will provide a potential therapeutic target to treat diabetes-related hepatic steatosis.
