Cancer Pain Control Before and After Palliative Care Center Referral

"الشعور بالألم يعتبر من اهم أنماط المعاناة التي يمر بها ما يقرب من 70% من مرضى السرطان خاصة مع تطور المرض وانتشاره في الجسم ولهذا يعتبر علاج الألم والسيطرة عليه واحد من التحديات التي تواجه أطباء الطب التلطيفي وذلك من اجل توفير حياة أفضل لمريض السرطان بعيداً عن المعاناة قدر المستطاع.
تعتبر منظمة الصحة العالمية العقاقير الأفيونية ركن أساسي لعلاج الألم المتوسط إلى الشديد وقد وضعت توجيهات عالمية لاستخدام هذه العقاقير بما يتناسب مع شدة الألم وبرغم ذلك فإن الكثير من مرضى السرطان ما زالوا يعانون من الألم الناتج عن الأورام وانتشارها بسبب مخاوف المرضى من الأعراض الجانبية لهذه الأدوية والخوف من الإدمان إلى جانب قصور معلومات بعض الأطباء عن تلك الأدوية.
من هذا المنطلق فقد أجريت عدة أبحاث علمية لدراسة مدى استخدام العقاقير الأفيونية بين مرضى السرطان وأكدت دراسات في الهند والمملكة المتحدة مدى قصور هذا الاستخدام في حين أوضحت دراسة أخرى في أوكلاهوما بالولايات المتحدة تداول هذه الادوية بصورة مرضية لمرضى الرعاية التلطيفية.
ولأن الجدال ليس محسوماً بعد في أمر مدى وصف الأفيونات لمرضى الرعاية التلطيفية فإننا بحاجة لمزيد من الدراسات في هذا الامر ولهذا فالغرض من هذا البحث هو معرفة التغيير في وصف المسكنات الأفيونية قبل وبعد التحويل الى مركز الرعاية التلطيفية.
وقد تم عمل دراسة استطلاعية لمعرفة مدى السيطرة على الالم عند مرضى السرطان اللذين بدأو العلاج التلطيفي من بداية شهر يناير 2016 الى نهاية ديسمبر 2017.
وقد تم تجميع المعلومات من المرضى انفسهم او ذويهم بعد موافقتهم على تسجيل درجة الالم قبل بداية خطط العلاج التلطيفي مباشرة في نفس يوم تدخل العلاج التلطيفي وبعد 3 ايام ثم بعد 6 ايام ثم بعد 14 يوم. وبالطبع تم تسجيل انواع المسكنات وجرعاتها والادوية المساعدة لتسكين الالم قبل بداية العلاج التلطيفي وبعد 3 ايام وبعد 6 ايام وبعد 14 يوم."

Abstract English

"Pain as a common symptom among cancer patients receiving palliative care services is progressively increasing at the end of life. Almost 70% of cancer patients experience pain during their course of illness, therefore palliative care team is concerned with providing better pain control to improve the quality of life of cancer patients.

Opioids are considered by World Health Organization (WHO) as the golden standard for the treatment of moderate to severe pain in the field of palliative care medicine.

Despite the introduction of many guidelines for treating of cancer pain especially WHO analgesic ladder, still cancer patients continue to suffer from pain, especially those who are terminally ill.

Many factors can contribute to ineffective cancer pain control such as fear from opioids side effects or addiction, as well as insufficient knowledge of physician about opioids in addition to patients' misconception about opioid medications.

Many studies have been done to assess opioid prescription in cancer patients. Some studies documented the under treatment of cancer pain related to low opioid prescription for cancer patients while other studies addressed a satisfactory opioid use and control by palliative care physician. As long as ineffective cancer pain management still a common problem especially among palliative care patients so more studies are needed in this field."
