Abstract English
Type 1 diabetes (T1D) is an autoimmune disease characterized by loss of beta cells in the pancreas; thus, exogenous insulin is the mainstay of its management. Currently, insulin is taken either in fixed doses for meals or variable doses via the carbohydrate counting method. Fixed insulin dosing regimens may result in hyperglycemia or hypoglycemia if the insulin dose is mismatched with the ingested meal. The carbohydrate counting method has been shown to be superior to fixed dosing; however, literature reveals a tendency to underestimate carbohydrate amounts when using this method.
To assist people with diabetes in making better decisions regarding their care, new technological tools have been introduced. Among these tools is a novel learning algorithm (LA) designed to optimize carbohydrate ratios for patients with T1D on multiple daily injection (MDI) therapy. Results showed that LA-driven changes were comparable to physician adjustments (PA): time in target glucose range (3.9–10 mmol/L) was 34.2% with LA vs. 36.9% with PA (p = 0.97), and the number of hypoglycemic events per day was similar (0.6 in LA vs. 0.8 in PA), with no cases of severe hypoglycemia or ketoacidosis. Another study using an app for carbohydrate counting via image recognition also showed positive outcomes, including reduced carbohydrate estimation errors and improved HbA1c levels.
Therefore, in this study, we aim to investigate the non-inferiority of simplified qualitative meal size estimation for insulin dosing compared to the carbohydrate counting method, through a mobile application designed to reduce the burden of carbohydrate counting for individuals with T1D on MDI therapy. The trial will follow a randomized controlled, cross-over design.
• Primary endpoint: Time-in-Range (TIR) as recorded by the Libre 2 sensor, and HbA1c levels in individuals with T1D on MDI therapy atDasman Diabetes Institute (DDI) (DDI) and Ministry of Health (MOH).
• Secondary endpoints: Time-Above-Range (TAR), Time-Below-Range (TBR), glucose standard deviation, mean sensor glucose level, and total insulin delivery in the same cohort.
